Інформація для булави

Тепер що Ви будете членом булави ПК, Вам потрібно здати до Адміністрації нищеподані документи:

  1. W-4 form: можна знайти тут.
  2. Медична форма: можна знайти тут.  Щоби виповнити форму, потрібно підпис лікаря, а не потрібно лікарський огляд якщо Ви кожного року це залагоджуєте.
  3. Інформації про страховку (insurance information): прошу відбити копію своєї картки медичну страховку і принести з собою на табір.
  4. Реєстрація авта: якщо хочете лишати своє авто на оселі підчас табору, прошу виповинити цю форму.
  5. Посвідчення першої допомоги/CPR (First Aid/CPR Certification): прошу відбити копію своєї картки посвідчення і принести з собою на табір.
  6. Посвідчення ступеня рятівника:  Якщо Ви здобули ступень рятівника, прошу відбити копію своєї картки посвідчення і принести з собою на табір.
  7. Відбитки пальців (Fingerprinting): Закони стейта Огайо вимагають що всі, які працюють при літних таборах, мусять здати свої відбитки пальців роботодавцеві.  Навіть якщо вже здали свої відбитки пальців минулого року чи для інакшої роботи, закон вимагає щоб Ви осібно здали кожного року і кожному роботодавцеві.  Процадура така:

а. Прошу подзвонити до поліції чи шерифа Вашої місцевості і поспитати чи вони перевіряють відбитки пальців для тих, які виконують працю на літних таборах.  Прошу або принести висліди перевірки з собою на табір, або вислати висліди перевірки на адресу: Pysanij Kamin, 17030 Shedd Road, Middlefield, Ohio 44062.  Тримайте свій рахунок, ПК Вам відплатить кошт.

б. Ті що приїзджають на табір з інакших стейтів (замість Огайо) чи

країн, мусять також здати перевірку “FBI check.” То можна здобути в той сам час як перевірка відбитки пальців.

в. Якщо Вам є неможливо виконати цю вимогу перед табором, можна поїхати до Шерифа Джіаґа, біля оселі.  Прошу повідомити подругу Марту Мудру (mudri@hotmail.com) якщо Ви плянуєте поїхати до Шерифа Джіаґа за відбитками пальців і FBI check.  Тримайте свій рахунок, ПК Вам відплатить кошт.

 

Можна всі форми вислати до оселі заздалегід табору (Marta Mudri, 17030 Shedd Road, Middlefield, Ohio 44062), або вислати е-поштою до ОТК (mudri@hotmail.com), або можна їх принести з собою на табір і здати Адміністрації тоді.

 

Now that you have been hired as part of the bulava at PK, there are some documents you need to file with PK.  These include:

  1. W-4 form: find this form here.
  2. Medical form: Please note that a physical is not necessary, as long as you have had a physical in the last year.  A physician’s signature is necessary.  Please find this form here.
  3. Insurance information: please make a copy of your insurance card and bring it with you to PK
  4. Car registration: if you intend to have a vehicle at PK during camp, please fill out this form.
  5. CPR/First Aid certification: please make a copy of your CPR/First Aid certification and bring it with you to PK
  6. Lifeguard certification: if you are certified to be a lifeguard, please make a copy of your Lifeguard certification and bring it with you to PK
  7. Fingerprinting: Ohio law requires that all camp personnel be fingerprinted.  Even if you have been fingerprinted last year or for another job, the state requires that you be fingerprinted separately for each year and each place you work.  Please follow these instructions for fingerprinting:
    1. Call your local police station or sheriff’s office and ask if they conduct fingerprinting for people who need to comply with camp safety laws.  Please have your fingerprint results mailed to Pysanij Kamin, 17030 Shedd Road, Middlefield, Ohio 44062 or bring them with you to PK.  Hold on to your receipt, as you will be reimbursed for the cost of fingerprinting.
    2. All out-of-state (non-Ohio) employees also need to get an FBI check – this can be done at the same time as your fingerprinting.
    3. If it is impossible for you to get fingerprinted prior to arriving at PK, you can get your fingerprinting completed at the Geauga County Sheriff’s Office near PK.  Please inform Marta Mudra (mudri@hotmail.com) that you will need to be fingerprinted at the Geauga County Sheriff’s Office.  Hold on to your receipt, as you will be reimbursed for the cost of fingerprinting.

All forms can be mailed to PK in advance of tabir (17030 Shedd Road, Middlefield, Ohio 44062), emailed to OTK (mudri@hotmail.com), or brought with you when you arrive for tabir.
